I authorise Shilleena’s Dance Academy Pty Ltd (“SDA”) to use the following Materials:
- Film photographs
- Digital photographs
- Motion picture footage
- Audio recordings
of me/my child in accordance with the following:
OPTION A — No Media Use
No Materials are to be produced featuring me / my child.
In this instance, I understand that I / my child will be unable to participate in SDA’s Performance Troupes, higher-level SFD examinations,
or the end of year concert.
OPTION B — Limited Media Use
No Materials are to be produced featuring me / my child except in the following instances:
- motion picture footage of the end of year concert performance,

digital photographs at both the rehearsal for the end of year concert and the two shows on concert day by a professional photographer,
- digital photographs taken by a professional photographer at a studio photo shoot prior to concert day, and
- In-class activities for at home practice posted in private WhatsApp groups moderated by the studio Director.
In this instance, I / my child will be unable to participate in SDA’s Performance Troupes or higher-level SFD exams.
By selecting this option, I am / my child is denying the Reproduction and Use terms of this Release.
OPTION C — Full Media Use (Promotional & Merchandising)
Materials will be recorded and used with the understanding that the content may be used for promotional or merchandising purposes. Materials
will be collected during class, exams, eisteddfods, rehearsals and performances. Materials will be tasteful and respectful.
Locations Where Media May Be Collected
Materials may be collected at or around the following locations:
- York Street Studios, Coorparoo
- Logan Entertainment Centre, Logan
- Eisteddfod/Performance locations throughout Queensland
Reproduction and Use
SDA has the right to photograph me / my child on any medium and the unrestricted right and permission to copyright, use, re-use, publish,
and republish the Materials in all forms of media (including printed materials) for art, promotional purposes (including, but not limited
to, advertising, commercial, or display use), illustration, exhibition, editorial, trade, or any other purpose in both domestic and foreign
markets.
SDA has the right to edit, delete and rearrange Materials in perpetuity, without any further requirement for consultation or consent from me
/ my child. This consent is given for the benefit of SDA and licensees, assignees and future owners of copyright in the Material of my / my
child’s performance.
Release of Claims
I waive any right that I / my child may have to examine, approve or demand the deletion or modification of the Materials. I release SDA of
any liability to me / my child for any infringement of my rights arising from the use of the Materials, including (but not limited to)
claims under tort law, contract law and the Competition and Consumer Act 2010 (Cth) and equivalent state and territory legislation.
I hereby release and discharge SDA and all persons functioning under their permission or authority (including, but not limited to,
associates, affiliates, officers, agents, advertising agencies, designated directors, employees, and customers) from any claims relating to
the use of the Materials including blurring, alteration, distortion, or uses in the process of recording, processing, or publishing the
Materials, including any claims for libel or invasion of privacy.
Consent Statement and Agreement
I have read the previous and fully understand the contents. I have full power to enter the release contained in this document and the terms
of this Release do not in any way conflict with any existing commitment on my / my child’s part.
For dancers under the age of 18 years, I represent that I am the parent or guardian of the above-named dancer.
